Hepatic resection is the standard treatment for early-stage hepatocellular carcinoma (HCC). However, its application in HCC patients at middle- and advanced-stage or with portal hypertension is controversial because of the high incidence of perioperative complications and mortality rates, and low long-term disease-free and overall survival rates. Here, the authors discuss the issues of these concerns.
