摘要
腹腔镜下经胆囊管胆总管探查取石术(LTCBDE)已被报道在择期手术中是安全有效的,但在急诊手术中的疗效尚未得到证实。因此,本研究探讨急诊LTCBDE治疗胆囊结石合并胆总管结石患者的安全性和可行性。
回顾性分析87例于2016年1月—2020年10月在合肥市第二人民医院接受LTCBDE治疗的胆囊结石合并胆总管结石患者的临床资料,其中37例急诊行LTCBDE治疗(急诊组),50例行择期LTCBDE治疗(择期组)。比较两组患者的一般基线资料、围手术期资料和随访资料。
两组患者的临床基线资料差异无统计学意义(均P>0.05)。两组均无中转开腹病例,均顺利完成腹腔镜手术。87例患者中72例顺利完成LTCBDE,15例改行胆总管切开完成探查取净石后行胆总管一期缝合。急诊组和择期组中分别有29例(78.4%)、43例(86.0%)完成经胆囊管探查,两组探查成功率差异无统计学意义(P=0.352)。与择期组比较,急诊组的手术时间延长(P=0.015)、术中出血量增加(P=0.014),其他围手术期指标差异均无统计学意义(均P>0.05)。两组共发生并发症6例,其中急诊组4例(2例胆汁漏、1例腹腔积液和1例结石残留)、择期组共2例(2例胆汁漏),两组的总并发症发生率及各单独并发症发生率差异均无统计学意义(均P>0.05)。
在接受腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)的胆囊结石患者中有10%~18%合并有胆总管结
胆总管探查有胆总管切开探查和经胆囊管探查两种方式。胆总管切开探查技术成熟,探查取石成功率高,但对胆总管损伤大,术后常放置T管,并发症较
对2016年1月—2020年10月期间在我院接受LTCBDE的87例胆囊结石合并胆总管结石患者的临床资料行回顾性分析。所有患者确诊均依据:超声、CT和(或)磁共振胆道胰管造影(magnetic resonance cholangiopancreatography,MRCP)。根据患者术前是否合并急性炎症(包括胆囊炎、胆管炎、胰腺炎)分为急诊组和择期组,急诊组为结合临床症状、体征、实验室和影像学检查,分别按《东京指南(2018)
纳入标准:⑴ 术前腹部超声、腹部CT、MRI胆管造影明确诊断为胆囊结石合并胆总管结石;⑵ 术前胆总管直径≥8 mm;⑶ 胆囊管直径≥4 mm;⑷ 胆总管结石直径<10 mm。排除标准:⑴ 合并肝内胆管结石患者;⑵ Mirrizzi综合征患者;⑶ 胆总管下端狭窄患者;⑷ 既往有上腹部手术史者;⑸ 疑有胆道肿瘤患者。
所有患者均实施气管插管全身麻醉后取头高足低左倾位。常规实施四孔法建立气腹,脐下方置10 mm套管针作为观察孔,注入CO2气腹压维持在14 mmHg(1 mmHg=0.133 kPa),30°腹腔镜直视下分别在剑突下、右锁骨中线肋缘下及右腋前线肋缘下2 cm置10、5、5 mm的穿刺孔。仔细解剖并暴露Calot三角,夹闭切断胆囊动脉,为防止胆囊结石位移至胆总管,分离出胆囊管后靠近胆囊处夹闭胆囊管,牵引胆囊充分暴露胆囊管,向胆总管方向进一步解剖游离胆囊管,暴露胆总管后,于胆囊管前壁,沿胆囊管纵轴T形切开,直至距胆囊管胆总管汇合处约3 mm,根据情况可适当延长,必要时可切开部分胆总管。分离钳扩张胆囊管后插入5 mm胆道镜探查胆总管并取石,胆囊管取石一般采用钢丝网篮、盐水灌洗等方法,必要时可用液电碎石清除结石。取净结石后再次探查胆总管,观察胆总管下端及Oddi括约肌的炎症水肿和收缩舒张情况,可用导丝探查胆总管远端和乳头的通畅性。确认胆总管无结石残留后,可吸收夹夹闭并切断胆囊管,如汇入部切开,可从切口最下端开始用倒刺线行全层连续缝合胆囊管至距胆总管约5 mm处后将胆囊管夹闭,切断胆囊管,切除胆囊。将胆囊和胆总管结石装入一次性标本袋,剑突下切口取出。如经胆囊管探查失败,则行胆总管切开探查取石。术后文氏孔附近常规放置负压引流管1根后关闭切口。术后当腹腔引流管无液体或引流量<25 mL/d时,将引流管拔除。
主要比较两组患者的探查成功率(为顺利完成经胆囊管探查比率,失败则改行胆总管切开探查)、术中出血量、手术时间、术后拔管时间、术后住院时间。两组患者术后胆汁漏、出血、切口感染、腹腔积液、结石残留、胆管狭窄、结石复发等相关并发症发生情况。其中胆管狭窄指术后反复发作胆管炎伴有梗阻性黄疸,B超或MRCP提示肝外胆管局限性狭窄伴;结石复发指胆总管结石取净6个月后胆总管再发结石。术后随访:所有患者术后随访半年以上,3~6个月至门诊随访一次。所有患者均行体格检查、肝功能、上腹部超声常规检查,根据情况必要时行腹部CT或MRCP检查。
两组患者在性别、年龄、肥胖指数(BMI)、美国麻醉协会(ASA)评分、胆囊管直径、胆总管直径、胆总管结石数、最大胆总管结石直径和合并症等临床基线资料比较均无统计学差异(均P>0.05),具有可比性(
本组87例患者中共有72例顺利完成经胆囊管探查取石;15例经胆囊管探查失败,改行胆总管切开完成探查取净石后行胆总管一期缝合。急诊组和择期组中分别有29例(78.4%)、43例(86.0%)完成经胆囊管探查,两组探查成功率无统计学差异(P=0.352),两组中无中转开腹病例,均顺利完成腹腔镜手术。急诊组较择期组的手术时间长(P=0.015)、术中出血量多(P=0.014);两组患者的其他围手术期指标比较差异均无统计学意义(均P>0.05):术后拔管时间(P=0.396)、术后住院时间(P=0.725)(
两组中总并发症共有6例,其中急诊组4例、择期组2例,总并发症差异无统计学意义(P=0.395);两组患者术后胆汁漏、出血、切口感染、腹腔积液、结石残留比较差异均无统计学意义(均P>0.05);两组中各有2例患者术后发生胆汁漏,4例胆汁漏患者每天引流量约30~110 mL,均无发热、腹痛等症状,保持引流通畅、延长引流5~14 d均治愈;急诊组中有1例发生腹腔积液,其患者术后腹腔引流管引流出黄褐色液体,每天约15 mL,第3天复查CT证实右上腹胆囊窝处包裹性积液,后在B超定位下成功穿刺引流出黄褐色液体约80 mL,术后8 d顺利拔管治愈;急诊组中有1例患者发生结石残留,该例患者术前MRCP检查胆总管直径较粗约16 mm,结石数多有7枚、结石直径均约5 mm,术后19 d出现腹痛,CT证实胆总管有结石残留后行ERCP顺利取出结石1枚。所有患者术后随访半年以上,随访期内未出现胆管狭窄和结石复发病例(

随着生活水平提高和饮食结构变化,胆总管结石的发病率逐年增
LTCBDE可充分利用胆囊管这一自然孔道,避免了胆总管切开,保护胆总管的完整性,与经胆总管切开探查相比,无需放置T管,避免了腹腔镜下的缝合,降低手术难度和缩短手术时间,经胆囊管探查术后与LC术后恢复过程相同,住院时间短、术后并发症少、恢复
LTCBDE手术成功的关键在于能否顺利完成经胆囊管探查,因此选择合适的病例尤为重要。已有研
本研究中急诊组较择期组的手术时间长、术中出血量多,这可能与急诊组患者的急性炎症造成胆囊及其周围组和肝十二指肠韧带炎症水肿、组织粘连,分离粘连导致手术时间增加、术中渗出增多有关,然而两组的术后拔管时间及术后住院时间无统计学差异,进一步说明急诊LTCBDE的有效性。急诊组的总体术后并发症发生率略高于择期组,但差异无统计学意义。最常见的主要并发症之一,共有4例(4.6%)胆汁漏发生,与相关文
综上所述,对于合适的胆囊结石合并胆总管结石急诊患者行LTCBDE是安全可行和有效的,和择期行LTCBDE相比,两组在探查成功率、术后拔管时间、术后住院时间和术后相关并发症发生率方面效果相当。急诊行LTCBDE可使部分胆囊结石合并胆总管结石患者避免胆总管切开及留置T管,手术损失小、术后恢复快、并发症少,可提高患者生活质量,在严格把握手术适应证前提下,值得临床应用和推广。
